    Five connected sheets

    Item setup
Base units, pack sizes, current ZMW prices and local usage rates.
    Usage log
Measured consumption by date, item and lot, with routine work, QC, repeats and waste kept separate.
    Lot register
Opening stock, receipts, outward transfers, expiry and physical-count differences.
    Summary
Usage, waste and costs from the start of the month through your chosen reporting date.
    Source catalogue
Original product references and historical prices for checking against current supplies.

    QUICK START

    Pilot on one bench

Start with 5–10 representative items for one month. Choose supplies your team can count or measure consistently.

    1. Set up your items

Give each item a unique ID and confirm its base and workload units. Create separate items for each AST agent and potency, identification panel or molecular assay.

    2. Confirm local values

Enter pack quantities, current ZMW prices and SOP-based usage rates for routine work, QC and repeats. Leave unknown values blank; enter zero only when it is correct. Historical prices do not populate current costs.

    3. Register your lots

Assign a unique lot ID for each item and manufacturer lot. Record expiry, opening and in-use discard dates, opening quantities, receipts and outward transfers. Enter known zero quantities as zero.

    4. Record daily use

Select the date, item, lot and work type. Enter the work count and measured quantity consumed. Record QC, repeats and waste separately. Supplies issued to a bench are receipts, not consumption.

    5. Review and reconcile

Set the reporting month and “As at” date on Summary. Update stock movements through that date and enter physical counts taken on the same date. Reconcile weekly and at month end; investigate differences and expiry flags.

    At the microbiology bench

    • One specimen may use several plates, and one isolate may use several discs. Item workload counts do not automatically equal patient totals.
    • Track dehydrated media and prepared plates separately. Keep a preparation record of powder used, usable plates produced and losses. Use a documented costing method to avoid charging the same material twice.
    • Follow approved procedures for QC acceptance, expiry and in-use discard. A stock balance does not establish that a reagent is suitable for use.

Enter and save your laboratory records in your downloaded workbook. This page does not store or sync stock records. The template has room for 40 items, 100 lots and 500 usage entries.
